How Do GLP-1 Medications Work
Mechanisms Behind GLP-1 Drugs
Semaglutide and Tirzepatide both belong to a class of drugs known as glucagon-like peptide-1 receptor agonists (GLP-1RAs), which mimic the GLP-1 hormone in the body that helps control insulin, blood glucose levels, and satiety.
The first way GLP-1 drugs act is on receptors in the hypothalamus, which results in a decreased desire to eat, says Laura Davisson, MD, a professor at West Virginia University School of Medicine and director of the Medical Weight Management Clinic at West Virginia University in Morgantown, West Virginia.
People have less hunger and fewer cravings, and say they no longer have as much ‘food noise’ when they are on these medicines, says Dr. Davisson.
The second place where the medications work is in the gastrointestinal (GI) tract, also known as the gut. The medicines slow down food movement through the GI tract, which makes people feel full longer as an additional method of helping with weight loss, Davisson says.
Tirzepatide and Weight Loss Outcomes
Tirzepatide is a newer-generation weight loss drug that also acts on the GIP hormone to help with conditions including type 2 diabetes and obesity.
A study of 1,879 patients with type 2 diabetes and a body mass index (BMI) over 25 found that participants on tirzepatide lost about 4 to 12 pounds more body weight at the end of nine months than those on semaglutide.
A slightly higher percentage of those on tirzepatide reported adverse side effects compared to semaglutide patients:
Nausea: 17 to 22 percent compared to 18 percent
Diarrhea: 13 to 16 percent compared to 12 percent
Vomiting: 6 to 10 percent compared to 8 percent
Is There a Typical Weight Loss Timeline on a GLP-1 Drug
“We historically have recommended that 10 percent weight loss over six months is a reasonable target for medical weight loss, but some people are losing weight faster on the GLP-1 medications,” says Davisson. These people are what some doctors call hyper-responders, she adds. That said, the pace of weight loss varies quite a bit from one person to the next.
Semaglutide Weight Loss Results
One study followed 1,961 participants who were overweight or had obesity but not diabetes over 68 weeks. The participants, who started from an average weight of 230 pounds with obesity (a BMI of 38), had average weight loss of 14.9 percent (or 34 pounds) when taking a once-weekly 2.4-milligram (mg) injection of semaglutide. In comparison, the placebo group had a 2.4 percent weight loss (5 pounds) at the end of the same time period.
Furthermore, one-third of the study’s participants who took the weekly semaglutide lost at least 20 percent (an average of 46 pounds) of their body weight by the end of 15 months. The study notes that all participants made healthy lifestyle changes in addition to taking the semaglutide.
Tirzepatide Weight Loss Results
In another study, 2,539 adults who did not have diabetes and were overweight or had obesity (participants either had a BMI of 30 or more, or 27 or more and at least one weight-related complication) took tirzepatide once per week for 72 weeks, at doses of 5 mg, 10 mg, or 15 mg. At the end of the study, participants who took 5 mg of tirzepatide each week lost approximately 15 percent of their body weight, 19.5 percent when they took 10 mg doses, and 20.9 percent when on 15 mg doses.

What Happens if You Lose Weight Too Quickly on a GLP-1 Drug
Why Rapid Weight Loss Can Be Harmful
Due to decreased appetite, slower digestion, and gastrointestinal side effects, people on a GLP-1 drug may feel full after eating only half to three-quarters of what they used to eat, says Dr. Troutman. “For some patients, [a GLP-1 drug] can really curb their appetite, which can create a very low caloric intake. This needs to be monitored as we do not want excessive weight loss, which can create excessive muscle loss” and other possible problems.
Although all weight loss includes some loss of muscle, losing weight rapidly makes it particularly challenging to maintain muscle mass. A doctor may decrease the GLP-1 dose if weight loss is happening too quickly, depending on the individual. In more extreme cases—when a person isn’t getting adequate calories or is missing key nutrients—the result may be both muscle loss (sarcopenia) and malnutrition.
“We take many factors into consideration when choosing a medication dose, including rate of weight loss. We also consider side effects, body composition including lean body mass, adequacy of nutritional intake, and lab results,” says Davisson.
How to Preserve Muscle Mass While Losing Weight
Curbing muscle loss involves incorporating high-protein foods into the diet and including resistance exercises in a daily routine. The amount of protein needed depends on body weight, says Troutman. As long as kidney function is normal, she recommends that GLP-1 users aim for 100 g of protein per day, with 60 g as the minimum target.
Some high-protein foods that can help reach this goal include lentils, fish, and lean meats like chicken breast, according to the U.S. Department of Agriculture. Protein sources that also provide fiber—such as whole grains, nuts, seeds, beans, and lentils—are smart choices. Fiber also offers multiple benefits, including helping to resolve constipation, a common issue for some users on weight loss medications.
Other Warning Signs of Losing Weight Too Fast
Some warning signs that weight loss is occurring too rapidly include gallstone formation, hair loss, and fatigue, says Troutman. Rapid weight loss may indicate insufficient caloric intake and should be treated as a red flag.
“It’s not just about losing weight quickly; it’s how did [my patients] lose weight?” she says. “I tell my patients that how they eat to lose weight is ideally how they will eat to maintain their weight long term. It has to be sustainable. So if they are not eating—which creates a significant caloric deficit for weight loss—that’s not ideal.”
What Else to Know About Taking a Weight Loss Medication
Besides knowing what healthy weight loss looks like while taking a GLP-1 medication, it's also important to understand a few other things before starting your treatment plan.
Weight Loss May Not Happen Right Away
GLP-1 medications need to be increased slowly over time, explains Davisson. For example, it can take at least six months to reach the highest dose of Zepbound. It may take even longer if someone has more side effects. Some people start losing weight before reaching the maximum dose, while others only see results after they get there.
If a person’s weight loss slows down, the doctor might raise the dosage. “Weight loss is different for each person and hard to predict,” says Davisson. “Once someone reaches the top dose after about six months, they usually keep losing weight for the rest of the year, and sometimes even longer. But there are always exceptions.”
You Might Notice Some Side Effects
GLP-1 medications affect the stomach and digestive system, which can lead to side effects like nausea, vomiting, worse acid reflux, diarrhea or constipation, and stomach pain or cramping.
“These symptoms may get worse when the dose increases but usually get better after a while. In my experience, people who still have these side effects after a couple of weeks usually stop taking the medicine. It would be unusual for someone to keep using it if they are vomiting or have diarrhea so much that they lose too much weight,” says Davisson.
It’s important to talk to your doctor about these possible side effects and what to do if they happen. Also, contact your doctor if you develop any health problems that aren't typical side effects.
You’ll Need a Plan to Keep the Weight Off
Once you reach your target weight, Troutman recommends working with your doctor to plan how to maintain it.
“Our bodies are naturally designed to regain weight, which can be frustrating. That’s why many people need to keep taking medication to stop the weight from coming back,” says Troutman. “Some people stay on the same dose and schedule. For others, we might lower the dose or space it out more. It really depends on the person.”
Troutman points out that keeping in touch with your doctor is very important for finding the best plan for you. It’s also a good idea to have a backup plan in case a large amount of weight comes back. Davisson adds that even if someone keeps using a GLP-1 medication long term, weight regain can still happen.
“This is because the body tries to protect its weight. When someone loses weight, the body’s natural responses, like increased hunger, can lead to gaining it back. That’s why it’s hard to keep weight off for a long time.”
One helpful way to maintain weight loss is to stay physically active. The Centers for Disease Control and Prevention (CDC) suggests all adults aim for:
150 minutes or more of moderate aerobic activity each week
- Two sessions of muscle-strengthening activities each week
The CDC also says that people who keep the weight off usually do 60 to 90 minutes of moderate activity on most days. This can be done in small chunks, like taking a brisk walk three times a day. GLP-1 Medications Are Not Meant for Short-Term Use
It’s important to avoid thinking of GLP-1 treatment as a quick solution. Instead, it will likely become a long-term part of managing weight — along with eating healthy, staying active, and drinking enough water.
“We now understand that obesity is a long-term medical condition. So using medication for a short time to treat something ongoing isn't the best strategy,” says Troutman. She adds that weight loss treatment should be viewed like treatment for other long-term issues such as high blood pressure.
For instance, can someone take blood pressure medicine for a few months and then quit? Usually not — the pressure would likely go back up. The same is true for weight and obesity.
Davisson agrees that using GLP-1 medications works best when it’s a long-term treatment. However, he also notes that research in this area is still growing.
“The general idea is to treat obesity as a chronic disease, which means using GLP-1 medications for a long time. But since we haven’t been using these drugs for many years yet, we’re still learning. Some people will stop using them, and that will help us study what happens. Maybe we’ll find that people can keep the weight off if their habits change a lot, or if there are other medicines that help with maintenance.”
